Job Description

Role: NHS Referral project caseworker
Salary: £25,295 FTE (pro rata, 37 hours FTE)
Hours:  25 hours per week ( 3 ½  days)
Responsible to: Partnerships & Inclusion Manager (and delegated staff)
Location: South Warwickshire (Stratford/Leamington office – currently remote working due to Covid19 restrictions on our service)
Duration: Till 31st March 2022 (possibility of extension)

Role Purpose  

To provide advice and advocacy support to vulnerable clients referred from South Warwickshire NHS foundation Trust (SWFT) & NHS social prescribers. Undertaking intensive casework support and assisted referral to other support organisations. Working across South Warwickshire depending on the project and clients needs.

South Warwickshire NHS Foundation Trust & Social Prescribers are able to refer service users residing in South Warwickshire who fall under South Warwickshire CCG in need of our advice services. As a caseworker you will work with the social prescribers and other organisations to help resolve issues that the client might have, with emergencies and priorities being dealt with first. You will be helping clients minimise rent arrears, income maximisation through benefit claims, hardship grants, improving financial capability, helping clients to resolve their debt issues, as well as trying to resolve other issues the clients might have.

Caseworkers provide supportive intervention and work as part of a multi disciplinary team. Above all else, you should have compassion and drive to help change people’s lives for the better and provide excellent quality of advice.
